I request my personal information held by the Parish of Tongariro Catholic. This request is made under Information Privacy Principle 6 of the Privacy Act 2020.

The personal information requested on this form will be used for the purposes of processing access requests only. This request form will be retained by the Parish Privacy Officer/Diocesan Archivist for a period not exceeding one year from the date of action, after which it will be securely destroyed. Copies of proof of identity provided will be securely destroyed immediately after verification. Where information is to be emailed to the requestor or representative, please note that the security of the email cannot be ensured. Despite our efforts, the data included in emails could be infected, intercepted, or corrupted. Therefore, the recipient should check the email for threats with proper software, as the sender does not accept liability for any access or damage inflicted by parties other than the sender and recipient.
